After
Hours Nurse Services
are provided 24 hours a day
through Team Health Medical Call
Center. The nurse can give you
advice about how to take care of
yourself at home, or tell you if
you need an appointment or need
to be seen in the Emergency
Room.
Lab
services are
available through Regional Medical Laboratories Inc. (RML)
at our Battle Creek site. This
substation is able to perform
immediate onsite testing, thus
eliminating the need for another
stop to have your blood work
done. RML accepts the Family
Health Center Sliding Fee
Discount Program. At our Albion
site, lab services are available
through Oaklawn Hospital.
Prescription services
are provided by Pharmacy Group
Practice Associates (PGPA) at
our Battle Creek location only.
For your convenience, 180 of the
most commonly used medications
are maintained on site in a
state-of-the-art dispensing
unit. PGPA accepts the Family
Health Center Sliding Fee
Discount Program.
Radiology services
(Albion site only) are available
at our Albion location through
Oaklawn Hospital.
Prescription assistance
may be accessed through the
sliding fee discount plan or by
our prior authorization process.
On-site application assistance
to apply for The Prescription
Drug Access Program connects you
to the pharmaceutical companies
offering free prescriptions to
patients who qualify. In urgent
situations, a voucher can be
given to a patient to cover
prescription needs. Seniors may
be able to qualify for the
Senior Millage Program for
prescription assistance.
Managed
Care services connect
our patients to other health
care providers ensuring that
those additional services and
referrals will be covered by a
variety of health plans.
Medical
Records organize and
maintain critical information in
patient files ensuring
continuity for the highest
quality of care. Incoming
medical reports are added to the
Electronic Health Record (EHR)
for immediate viewing within the
system and review by the
provider.
At Family Health Center of
Battle Creek and Albion, we are
sensitive to the private nature
of a patient’s medical records.
We take steps to assure that
your records are kept
confidential and secure. Medical
information is not released
without your written permission
on our authorization form.
Copies of records for other
health care providers are made
free of charge. There is fee for
copies made for your personal
records. A separate
authorization is required for
each request.
Forms
Completion assistance
is available through our Health
Information Services Specialist.
She will assist you in getting
your forms, such as disability
or FMLA, completed and answer
your questions about the
process.
Patient
Service Center has
nurses available during regular
office hours to talk to patients
by phone or on a walk-in basis.
They provide assistance at the
time of the call, regarding lab
results, prescription refills,
patient/provider liaison and
many other concerns.
Patient
Accounts generates
reliable and prompt billing with
sliding fee discount adjustments
for qualified patients. The
sliding fee discount is based on
household size and income.
Medical plan qualification
assistance is offered to
patients for enrollment in
MIChild and Medicaid.
Receptionists/Schedulers
handle over 800 calls per
day, schedule over 300 patients
per day and greet each patient
with warmth and respect. Our
open access model allows for
same day appointments for ideal
patient service.
Therapy
Pet brings
encouragement to patients who
may be frightened or hurting at
their doctor appointment.
Barnabas (labradoodle) is
available to entertain and calm
patients of all ages.
Translation Services
provide full time Spanish
translation so Spanish speaking
patients can access every
service offered at Family Health
Center. Other language
translation needs are by
arrangement. Sign language
interpreter is provided as
needed, as well as a TDD machine
on site.
Transportation assistance
can be in the form of; bus
tokens, taxi requests, gas
vouchers, or through the Senior
Millage Transportation Program.
The
Food Pantry provides
non perishable food items
obtained through the local SW
Michigan Food Pantry for
patients in need. Employee
donations of canned and boxed
goods, household supplies, baby
items and winter coats are also
available.
