“Open Access System”: One of

Health Center of Battle Creek is
our open access appointment
scheduling system in which same
day appointments are available.
Arrive
early: Please allow extra
time to register at your first
visit. If you need help with
insurance coverage or billing
arrangements, you should also
allow additional time to speak
with one of our Patient Account
Representatives.
Please
bring with you to every
appointment:

Photo
ID and
insurance cards, copies will be
made for our records.

A list of all your
current medications, dosage and
how often you take the medicine.
When you
check In: You will be asked
to verify your current address
and telephone number. It is
very important that we have this
information in case we need to
contact you to follow-up on a
medical problem. Please be sure
to let us know of any changes.

call to cancel or reschedule
if you are unable to keep your
scheduled appointment.
